Sugar Alcohols and Flatulence: Why Sugar-Free Means Gas-Full
Sugar-free doesn't mean consequence-free — the polyols that sweeten "diet" candy and chewing gum are notorious for producing impressive amounts of intestinal gas.
What Are Sugar Alcohols?
Sugar alcohols, known to chemists as polyols, are a class of carbohydrate-derived sweeteners that include sorbitol, mannitol, xylitol, maltitol, erythritol, and isomalt. Despite the name they contain neither table sugar nor drinking alcohol; the term refers only to their molecular structure, in which a hydroxyl group replaces the carbonyl group found in ordinary sugars. They occur naturally in small amounts in fruits such as pears, apples, and prunes, but most of the polyols in modern diets are manufactured industrially and added to products labelled "sugar-free" or "no added sugar." They are popular because they deliver roughly 40 to 90 percent of the sweetness of table sugar while contributing fewer calories and having only a minimal effect on blood glucose, which makes them attractive for diabetic, low-carbohydrate, and tooth-friendly foods.
Why They Cause Gas
The digestive trouble with sugar alcohols comes down to a simple fact: the human small intestine absorbs them poorly and slowly. Because their molecular shape resists the enzymes and transporters that handle normal sugars, a large fraction passes undigested into the large intestine. There, two things happen at once. First, the unabsorbed polyols are osmotically active, drawing water into the bowel and frequently producing a laxative effect — which is why labels warn that excessive consumption may loosen the stool. Second, the resident gut bacteria ferment these molecules eagerly, releasing hydrogen, carbon dioxide, and methane as by-products. This bacterial fermentation is the direct source of the bloating, cramping, and flatulence that follow. Erythritol is the notable exception: its small molecule is absorbed in the small intestine and excreted in urine, so it produces far less gas than its cousins.
Where You'll Find Them
Sugar alcohols hide in a surprising range of everyday products. Sugar-free chewing gum and mints are leading culprits — a single stick of gum may contain a gram or more of sorbitol or maltitol, and a habitual chewer can easily cross the threshold for symptoms. Polyols also appear in diabetic chocolate, "keto" and protein bars, sugar-free hard candies, reduced-calorie ice creams, cough drops, and even some toothpastes and mouthwashes. Because regulations in many countries do not require the polyol content to be flagged prominently, consumers often have no idea why an innocent handful of sugar-free sweets has left them doubled over. Scanning ingredient lists for words ending in "-ol" — sorbitol, maltitol, mannitol, xylitol, isomalt — is the most reliable way to spot them before they spot you.
Managing the Effects
Tolerance to sugar alcohols varies enormously between individuals and depends heavily on dose. Most people can handle around 10 to 20 grams of sorbitol or 20 grams of xylitol per day before significant symptoms appear, but those with irritable bowel syndrome or a sensitive gut may react to far less. The practical advice is straightforward: read labels, introduce polyol-containing foods gradually, and keep the total daily intake modest. Erythritol and, to a lesser extent, isomalt are generally the best-tolerated options for people prone to gas. Spreading consumption across the day rather than eating a large amount at once also helps, since it gives the small intestine more chance to absorb what it can. If symptoms persist, switching to non-polyol sweeteners such as stevia, or simply moderating sweet intake, usually resolves the problem.
