What Is the Hydrogen Breath Test?

The hydrogen breath test is a simple, non-invasive medical examination that measures the amount of hydrogen (and often methane) gas in a person's exhaled breath. The logic is elegant: human cells cannot produce hydrogen gas at all. Every molecule of hydrogen you breathe out was manufactured by bacteria living in your digestive tract as they ferment carbohydrates. Some of that gas is absorbed through the gut wall into the bloodstream, carried to the lungs, and exhaled. By feeding a patient a measured dose of a specific sugar and then sampling their breath every fifteen to thirty minutes for two to three hours, clinicians can watch fermentation happen in real time. A sharp rise in breath hydrogen reveals that sugar reached gut bacteria instead of being absorbed normally. The test is painless, requires no needles, and has become one of the most useful tools in gastroenterology for explaining chronic gas and bloating.

The Science: Why Bacteria Put Hydrogen on Your Breath

Carbohydrates that escape digestion in the small intestine become food for the trillions of microbes in the colon. As these bacteria break sugars down without oxygen, a process called anaerobic fermentation, they release hydrogen, carbon dioxide, methane and other gases. Most of this gas leaves the body as flatulence, but a meaningful fraction diffuses across the intestinal lining into the blood. Because hydrogen is tiny and chemically inert in the body, it travels untouched to the lungs and is breathed out within minutes. This creates a direct chemical signal: the more fermentable sugar reaches the bacteria, the more hydrogen appears on the breath. Some people instead harbour methane-producing archaea that consume hydrogen and release methane, which is why modern analysers measure both gases. Reading the timing and height of the gas curve lets specialists distinguish normal digestion from malabsorption with surprising precision.

What the Test Can Diagnose

The hydrogen breath test is used to investigate several common but frustrating conditions. With a dose of lactose, it can confirm lactose intolerance: if breath hydrogen spikes, the milk sugar was not broken down and instead fermented in the colon. A fructose challenge can reveal fructose malabsorption, a frequent cause of bloating after fruit, honey or sweetened drinks. Using glucose or lactulose, doctors test for small intestinal bacterial overgrowth, or SIBO, where bacteria that belong in the colon colonise the small intestine and ferment food far too early, producing an abnormally fast rise in gas. The test can also estimate how quickly food travels through the gut. For millions of people with irritable bowel syndrome or unexplained flatulence, a breath test offers an objective explanation where symptoms alone are ambiguous, guiding targeted dietary changes instead of guesswork.

How to Prepare and Take the Test

Accurate results depend heavily on preparation, because anything that changes gut fermentation can distort the curve. Patients are usually asked to avoid antibiotics and laxatives for one to four weeks, since these alter bacterial populations, and to skip slowly fermenting foods such as beans, bran and high-fibre meals the day before. An overnight fast of eight to twelve hours is standard, and smoking and even vigorous exercise are discouraged on test day because both can change breath gas. At the clinic, a baseline breath sample is collected, the patient drinks the test sugar dissolved in water, and further samples are taken at regular intervals. The whole process takes two to three hours of mostly sitting and waiting. Results are plotted as a graph, and a rise above a defined threshold, judged together with any symptoms recorded during the test, points the clinician toward a diagnosis.
