Flatulence with Age: Why Older People Fart More
Many bodily functions change with age — and the gut is no exception. Many people notice from their fifth or sixth decade that flatulence becomes more frequent, certain foods are less well tolerated, and symptoms are perceived more intensely. Behind these observations lies a complex biology: aging gut muscles, an altered microbiome, declining enzyme production, and structural changes to the digestive tract.
Slowed Intestinal Peristalsis: The Main Factor
Intestinal peristalsis — the coordinated muscle movements that push food through the digestive tract — measurably slows with age. The smooth gut muscles lose elasticity and contractile strength, the neuronal innervation of the gut decreases (the nerve cells of the enteric nervous system become less numerous with age), and the balance of signaling molecules regulating peristalsis shifts. The result: food and gut contents remain longer in the large intestine — more time for bacterial fermentation — more gas production. The transit time through the large intestine (colonic transit time) increases in 60-year-olds compared to 30-year-olds by an average of about 20–30%.
Altered Gut Microbiome: Less Diversity, More Gas
The gut microbiome changes considerably over the lifespan. In older people, the diversity of gut bacteria typically decreases, while certain groups increase disproportionately. Particularly relevant: the amount of gas-producing Bacteroidetes bacteria often increases, while protective Bifidobacterium species (which produce less gas) decrease. At the same time, the production of short-chain fatty acids (SCFA) changes — substances that actually partially neutralize intestinal gas. Studies with over 1,000 subjects from various age groups show consistent changes in gas production profiles: older people produce more methane (correlated with slower intestinal transit) and more frequently have altered H₂S levels.
Declining Enzyme Production: Food Intolerances Emerge
Digestive enzyme production decreases with age — particularly noticeable with lactase (for milk sugar), lipase (for fats), and various amylases (for starch). Lactase deficiency often develops only in adulthood and increases over the years: estimates suggest that over 60% of Europeans over 60 have clinically relevant lactose maldigestion — many without being consciously aware of it. Fats are processed more slowly and remain longer in the small intestine, altering fermentation. The result: foods that posed no problem at age 30 can cause considerable flatulence at age 65. This is not personal failure but normal aging physiology.
Medications as Flatulence Amplifiers in Old Age
Older people statistically take more medications — and many common medications can have flatulence as a side effect. Metformin (diabetes medication) is one of the most frequent triggers of intestinal flatulence, with an incidence of up to 30% of patients. ACE inhibitors (blood pressure medications) can cause an irritating cough leading to increased air swallowing. Opioids (pain medications) drastically slow intestinal peristalsis. Antidepressants affect serotonin metabolism in the gut. Proton pump inhibitors (antacids) alter the pH value of the digestive tract and thus the environment for gut bacteria. A regular medication review with a doctor is therefore particularly sensible for flatulence problems in old age.
