Activated Charcoal, Simethicone & Co.: What Really Works Against Flatulence
The pharmacy shelves are lined with dozens of products promising to reduce flatulence or neutralize odor. From activated charcoal to simethicone to enzyme supplements — the market for anti-flatulence remedies is large. But what actually works, and on what mechanism is it based? Science has clear answers: some active ingredients are well-evidenced, others are pharmacologically questionable, and some home remedies outperform the majority of over-the-counter products.
Simethicone: The Best-Established Active Ingredient
Simethicone is the most effective and best-evidenced substance for flatulence complaints available over the counter. It is not a true pharmaceutical agent in the classical sense — it is not absorbed and does not alter gas production. Instead, simethicone is an antifoaming agent: it reduces the surface tension of small gas bubbles in the gut, causing them to coalesce into larger bubbles that can be expelled more easily as flatulence or belching. Clinical studies confirm a significant reduction in subjective flatulence complaints in postoperative gas accumulation and irritable bowel syndrome. Simethicone is remarkably safe (including for infants), has no known drug interactions, and is completely eliminated by the body.
Activated Charcoal: Effective Against Odor, Not Against Gas Volume
Activated charcoal (medicinal charcoal) has an extremely porous structure with a specific surface area of up to 3,000 m² per gram. It adsorbs (binds to its surface) a wide range of organic molecules, including the odor-active compounds in intestinal gas such as hydrogen sulfide, methanethiol, and dimethyl sulfide. Activated charcoal capsules can therefore reduce the odor of intestinal gas — but they do not reduce the gas volume itself. A controlled study showed that activated charcoal supplementation can reduce the H₂S content in flatus by around 70%. The caveat: activated charcoal adsorbs non-selectively — it also binds medications and can impair their effectiveness. At least 2 hours should be left between taking activated charcoal and other medications.
Alpha-Galactosidase: The Enzyme-Based Remedy
Alpha-galactosidase (in products such as Beano®) is an enzyme that breaks down the oligosaccharides in legumes (raffinose, stachyose) before they reach the large intestine and are fermented there. Studies show a reduction in intestinal gas production after legume-containing meals of 25–50% when taken before eating. Important: alpha-galactosidase works preventively — it must be taken before the gas-producing meal, not afterward. Lactase enzyme supplements work on an analogous principle for lactose-intolerant individuals. A beneficial effect also occurs without supplementation: soaking beans overnight and discarding the soaking water removes up to 70% of soluble oligosaccharides.
Bismuth Subsalicylate and Peppermint Oil: Underestimated Helpers
Bismuth subsalicylate (active ingredient in some stomach remedies) reacts with hydrogen sulfide in the gut to form odorless bismuth sulfide — a unique chemical approach that directly neutralizes the odor. Studies demonstrate a reduction in H₂S concentration in flatus of up to 95%. The downside: bismuth turns the stool black (harmless but alarming) and should not be taken long-term. Peppermint oil (in enteric-coated capsules) relaxes smooth intestinal muscles and reduces spasms — for IBS-associated flatulence, it is well evidenced as therapeutic. Carminative herbs such as fennel and anise have limited evidence in their botanical form, but are harmless as tea preparations and are subjectively perceived as effective by many.

Fun Fact
In 1998, British inventors developed underwear with a built-in activated charcoal pad — 'Flatulence Filter Underwear'. The product was presented at a health fair in London and received enthusiastic press coverage. The manufacturer received orders from 40 countries within a week. According to an interview with the inventor, the greatest challenge was not the technology but finding a suitable product name — 'Fart Filter Pants' was rejected by most retailers.
